The patient portal of Zoelife psychiatric services provides secure online 24/7 access from your mobile device or computer to access records. First, you must receive a validation code from our office prior to logging in.
We also use Spruce for patient communication with the office. It is HIPPA compliant, and we will send a link for download to all established patients.
Cancellation policy is as follows: Please give a 48 hours’ notice. You can reschedule or cancel your appointments by sending us an email. No-shows or missed appointments without 24-hour notice: $100 fee before you will be allowed to reschedule. Exceptions for true emergencies (family and medical emergencies).
This is not meant to be punitive, with less than 24-notice, the appointment time is not able to be filled by another patient, and 24 hours before your appointment, time is spent preparing for your appointment.
We are required by law to maintain the privacy of our patients’ protected health information and to provide patients with notice of our legal duties and privacy practices with respect to protected health information. We are required to abide by the terms of this Notice for as long as it remains in effect.
